Alright, I’ll be real with you, I never planned on going through the hell that is the One-Eyed Ghoul questline twice. But curiosity (and maybe a bit of ego) got the better of me. I mean, come on, who doesn’t want to become a regen tank with busted damage and drip to match? If you’ve played GHOUL://RE for even five minutes, you know this game doesn’t hold your hand. And if you’ve tried to become a One-Eyed Ghoul, chances are you’ve already been lied to about needing 100k rep, 30 grips, or some other outdated nonsense. So I made this guide, for people like us. The grinders. The curious. The borderline insane. I’m laying everything out clean: the real steps, real requirements, and real tips to help you unlock the most broken (and honestly, the coolest-looking) form in the game.
IMPORTANT: Please, for the love of all things Kagune, do this in a freshly updated server (Version 8878). Trust me, nothing hurts more than sweating through a full grind only to realize the server’s outdated and none of it counted.
How to Get ONE-EYED Ghoul
In Ghoul://RE, Ghouls are no joke. They’ve got regen, Kagune, and raw power that makes early game CCGs cry. But One-Eyed Ghouls? They’re the hybrids, like Half-Human Ghouls. Born of human and ghoul, they’re rare, OP, and honestly, some of the coolest characters in the Tokyo Ghoul universe.
In-game, unlocking One-Eyed Ghoul is like signing up for New Game++. You’re basically asked to max out, wipe, then do it all over again, but with a twist. You get boosted XP, rep, and eventually walk away with one of the most powerful builds in the game. It’s exhausting, it’s confusing, and yeah, you’ll probably rage once or twice. But trust me, once you see that regen in action? You’ll forget the trauma (kind of).
So, here’s everything you’ll need to do. No fluff, no outdated 100k rep rumors. Just pure grind facts.

Step 1: Max Out Your CCG Slot
Before anything, you need to finish your CCG path completely. That means:
- Max Level
- At least 75k+ Reputation (realistically 80k to be safe)
- Special Investigator Rank
- 15–20 PD Grips
- Arata Proto or Quinx unlocked (I recommend you just go with Arata though, so you don’t have to go through all the grind again)
Don’t listen to people saying you need 100k rep or 30 grips. I did it with 78k and around 18 grips, worked just fine. Just make sure you’ve hit a “complete” point in your progression.

How to Get Arata and Quinx
To get Arata and Quinx in Ghoul://RE, you’ll need 75,000 reputation (not 500k like people claim), eat 100 Cognitive Sacs in two batches after starting the quest by selecting “Powers” from the NPC’s second dialogue, collect 5 One-Eyed Kakuja Fragments, and have 500,000 yen.
Once prepped, head to the Black Reaper NPC to teleport to the Arata boss fight. The easiest way to beat it is by cheesing: equip Owl, go full strength (50), load up on damage gear, and spam ranged crits while staying at a distance. After winning, you get Arata, you can then roll your Quinx at the Ghoul Point Exchange by selecting Quinx Kagune Type Reroll. Moves use Y/Z for Kagune and X/C for weapon skills. It’s totally doable with the right setup.

Step 2: Wipe During a Perma Death Event & Choose Human Again
Once you’ve hit those benchmarks, head over to Vaz at the Anteiku café. If he tells you: “Over 30 sparks cling to your hands… When the delirium arrives, brush the slate clean.” Then you’re good to go.
If you get: “Additional Delirium can be acquired…” Then keep grinding. You need more PD grips.
Once you’re done, wipe that CCG slot during a Perma Death event. And yes, that means die. Not metaphorically, actually die. But make it count, because you’re about to start the journey toward becoming a One-Eyed Ghoul.
After wiping, you’ll get a weird little prompt like: “Something inside you doesn’t want to choose that path…” When you see that, you’re doing it right. Instead of going Ghoul, choose Human again. Then you’ll get a message saying something like: “A strange sensation washes over you… find the café…” or something like “You hear a whisper in your mind… Find the café—the one she frequents.”
Boom. That means the quest is officially active.

Step 3: Start the Secret Quest
As a normal, unaffiliated Human, go talk to Lezi at café. Do NOT be a CCG again at this point or you’ll mess it up and have to wipe again. No fun. Just go through her dialogue options, it doesn’t matter what you pick. Lezi will hint at her past and give you the questline to follow.
Once you do that, now you can become a CCG member. That starts your actual One-Eyed Ghoul progression.

Step 4: Grind Again & Reclaim What’s Lost (With Boosts)
Now you’re back at square one, but here’s where the magic kicks in: As a One-Eyed Ghoul in progress, you get 2.5x XP and rep. That means your grind back to max is way faster. But here’s the key: You need to hit the same milestones you had before you wiped.
If you had 90k rep, get back to 90k. If you had Arima, get it again. If you had Arata, you need it again too. Pro tip? Store or swap your Mythic/Legendary weapon before wiping. Trade it for something like Yukimura, something basic and craftable, so you can quickly re-equip and focus on your Arita or Quinx.
There are three major tasks:
- Reclaim Your Old Weapon – You have to get the exact same weapon you had before wiping. If it was Solace v1, get Solace v1 again.
- Tip: Wipe with a common starter weapon to make this part easier.
- Reach Peak Again – Basically, repeat the CCG grind.
- Unlock Arata/Quinx again
- Hit Max Level again
- Reach or surpass your previous Reputation and PD Grips
- Talk to Lezi Again – After proving yourself all over again, Lezi will finally point you to Dr. Kanou.

Step 5: Enter Kanou’s Lab
Dr. Kanou’s lab becomes accessible during a Perma Death event. Head to the CCG Elevator, and you’ll see an extra button labelled ??? (only appears if you’ve progressed far enough). That’ll teleport you straight to his lab.
Important: You can’t get in unless Lezi sends you.
What You Get
Here’s what comes with being One-Eyed Ghoul:
- A Legendary Kagune right off the bat
- A new Legendary Ghoul Clan (even if you had Kaneki before)
- Your custom clothes, white/black hair, etc. get re-applied
- You basically become a hybrid of your old self, Ghoul edition
No need to reroll unless you really want to. They give you all the heat upfront.
Wrap-Up: Is One-Eyed Ghoul Worth It?
Look, I’m not gonna lie. This quest is insane. You have to max out CCG twice on the same slot. That’s hours of grinding, wiping, regrinding, praying you don’t mess it up, and dealing with randoms trying to jump you. And no, I don’t think Kaneki ever had to go through this in the anime. But hey, game logic.
Is it worth it though? Absolutely. The regen is busted, especially when paired with Kakuja. I went into ranked PvP with it, and the thing kept me alive through stuff I should’ve definitely died from. I popped Kakuja and was like, “Oh, so I just don’t take damage now.” Even if you’re garbage at PvP (like me), this build carries.
