First, you’ll want to head towards the Vestige of Blind Agatha in Pilgrim’s Perch Bellroom. Once you are there, turn left and go deeper into the area.
Keep following along the path in the cave and you will reach a small pond. When you’re there, enter Umbral to reveal the path.
After that, keep following the path to your right. As you go along the path, there will be monsters standing in the middle of your way.
You may either choose to defeat those monsters, or simply choose to ignore them and keep heading deeper in.
After you get out of the cave area, you’ll be able to see some staircases that will lead you upwards.
Once you have reached the next floor, keep going until you are outside. There are also some monsters here as well, so try to either deal with them or just ignore and avoid their attacks.
Once you are out of the cave, climb down the ladders and keep sticking to the right side. You will eventually see yourself at a cliff.
Then, soulflay the two stigmas in front of you and you’ll be able to move the platform, which will act as a bridge to help you go to the other side.
Once you have reached the other side, soulflay the last stigma and the Relic of Perpetuation will drop.
You can then pick it up to add it to your inventory. This item will help you increase your maximum health. Health is a pretty universal stat, so it will be really useful for you in most of the situations.
