There’s been a recent trend of games having hidden achievements that can be obtained through various methods. You just have to know how the devs think when it comes to choosing a hiding spot that no one in their right mind will probably think of visiting.
This is the part where you’re forced to go back to Herta Space Station once more. Regardless if you’ve already seen everything the station has to offer in Honkai Star Rail, there’s always going to be a reason to come back, which is the recent Update 1.6.

Chronic Ailment Diagnostic Manual Hidden Achievement Guide
Chronic Ailment Diagnostic Manual is an Achievement in the category “The Memories We Share.” In order to get this achievement, you, the player, will have to collect all 4 parts of Dr. Ratio’s Note.
All four pieces of the note have been scattered around the various sections of the Herta Space Station. To be even more specific, you can pick up all of the notes in the Seclusion Zone.

First Note
To get things started, go to the room below the 2nd floor of the Seclusion Zone. The note will be on the middle table next to a computer monitor.
Second Note
Moving forward to the second note, teleport into the incubator. That will be on the first floor of the Seclusion Zone.
The note has been left haphazardly on the ground. Look for a faint glimmer if you’re having a tough time locating the note as the area is needlessly massive.
Third Note
Go and use the Pharmaceutical Room space anchor. Once you load into the area, go around the other side.
There’s a desk there with a giant plant box next to it. The note will be sitting on the left side of the two monitors.
Fourth Note
While still in the Seclusion Zone, go to the Breeding Grounds last. That’s on top of the second floor, exit the room passage and look to your right.
The last note will be right underneath Ruan Mei’s Creation Salty Rimeplume wedged in between a few boxes.
And that’s how you get the Chronic Ailment Diagnostic Manual achievement in Honkai Star Rail. You’re going to read all four notes now that you have them, right?
