Once you get to the item that you will want to upgrade you will just see an Upgrade option at the bottom. You will need to hold the SQUARE button and then assuming you have the right resources and gold you will be upgrading your weapon or armor.

First of all, though you must make sure that the weapon or armor is actually upgradable. There are some that you cannot upgrade at all. So just look for the text written on the right side of it and this is how you can upgrade it.

Every weapon and armor will have a couple of ranks that you can upgrade in to. The bigger the level the more resources that you will need. Also, each rank of the upgrade will bring you more damage, strength, element, HP, Defence, Vitality, and so on. It is highly recommended that you upgrade your gear in this game!
